The main goal of an endodontic treatment is complete removal of bacteria, their byproducts, and pulpal remnants from infected root canals and the complete sealing of disinfected root canals. Intracanal medicaments have been thought as an important step in killing the bacteria in root canals; however, in modern endodontics, shaping and cleaning has been emphasized greater importance than intracanal medicaments as a means of disinfecting root canals. Furthermore, biocompatibility and stability are essential properties for intracanal medicaments. The more modern meaning of intracanal dressing is for a blockade against coronal leakage from the gap between filling materials and cavity wall.

Antibiotics are an extremely valuable addition to the armamentarium available to health practitioners for management of bacterial infections.

Due to the potential risk of adverse systemic effects of systemic applications and ineffectiveness of systemic antibiotics in the necrotic pulpless tooth and the periradicular tissues, local application of antibiotics may be a more effective mode for delivering antibiotics to infected root canals.

The purpose of this paper is to discuss critically the antibacterial efficacy of intracanal medicaments on bacterial biofilm. The selected articles were identified from titles, abstracts and full-text articles by two independent reviewers, considering the tabulated inclusion and exclusion criteria. Disagreements were resolved by consensus. The search retrieved 91 related articles, of which 8. Intracanal medicaments were found to have a limited action against bacterial biofilm. The principle of a treatment to reach favorable outcomes in endodontic infection management requires the recognition of the problem and the removal of the etiological factors.
